Parent Consent Form - Individual Tutoring:

• this form must be downloaded and completed by tutors during their first session of one-on-one or two-on-two tutoring. The student must then be instructed to take the form home, have it signed by his/her parents and return the form to you at the next tutoring session. You can then either fax, scan in, or personally deliver this form to your Tutor Manager.

• if your student is over 18 years old then they may simply complete it themselves during their first session with you.

Tutor Report Form:

• tutors involved in either one-one-one or two-on-two tutoring are requested to download and complete one of these forms once per semester

• it should take at most 10-15mins to complete. Completed forms can be emailed as an attachment to your Tutor Manager.

• Your Tutor Manager will inform you in advance when the next report is due.
